Public Health Nurse (PHN) 

Cuyahoga County Board of Health

3 POSITIONS AVAILABLE

Reports to: PHN Supervisor

Position to be filled: January 3, 2023                                 

Salary:  $47,073.00 annually

Hours:  Mon. – Fri., 8:30 a.m. to 4:30 p.m. (37.5 hours per week)                      

Flexible Work Arrangements: Alternate work schedule or changes in work schedule available after initial training period with approval. Core work hours of 10am to 2pm required of all staff. Remote work from home available up to two days per week with approval after the completion of a minimum of 60 days of employment.

Vaccine Policy: Effective November 1, 2021 all new hires will need to meet COVID and Influenza vaccine requirements or request an exemption as outlined in the CCBH Vaccination Policy

Minimum Requirements

  1. Must be licensed registered nurse pursuant to Section 4723.13 of the Ohio Revised Code. 
  2. A valid Ohio driver's license with ongoing proof of auto insurance. 
  3. Current CPR certification.
  1. Ability to effectively function as a member of an interdisciplinary service team.
  2. Effective written and oral communication skills.
  3. Proficiency in Microsoft Office Suite.

General PHN Responsibilities:

May include but not limited to:

  1. Maintain accurate records and submission of required reports.
  2. Educate families and health care providers regarding programs offered in Prevention & Wellness Services.
  3. Attend interagency and interdisciplinary meetings as assigned.
  4. Perform other duties as requested by Supervisor or Service Area Director/Deputy Director.
  5. Participate as needed or requested in the event of any public health emergency.

Children with Medical Handicaps Program Responsibilities (2 positions):

  1. Provide case management for families with children with special healthcare needs.
  1. Link families to a network of providers and assist with obtaining payment for the services the children need.
  2. Assist with health system navigation and referrals.
  3. Provide annual home visits to families.

Newborn Home Visiting Program Responsibilities (1 position):

  1. Provide home visits for children and families.
  2. Conduct health and developmental screenings.
  3. Develop and implement health care plans compliant with program guidelines and protocols.
  4. Coordinate and document referrals to the appropriate resources.
