Service Coordinator

Greene County Public Health

 

Job Duties:  Successful candidate will work with families in their home or community environment to coordinate developmental evaluations for their child or children ages 0-3 years. Service coordinator will conduct family assessments, develop Individual Family Service Plans (IFSP), assist families in meeting their individual needs as identified in the IFSP, and ensure early intervention services are delivered in accordance with the child's or children’s IFSP. Service coordinator is required to maintain child files in accordance with program guidelines, document all activities, develop, and maintain relationships with early intervention providers and work with families to provide increased parent knowledge of child development, and/or home intervention services. The service coordinator will provide virtual, or in-person home visits as required by the program, coordinate services to the family and become a major support for the parent(s). All health district team members have an emergency response role and may be expected to respond to threats to the public’s health in accordance with the Greene County Public Health Emergency Response Plan.

Minimum Qualifications: 

Minimum associate degree from an accredited college or university in audiology, child and family studies, child development, child life, counseling, education, hearing and speech sciences, human development, human ecology, human social services, medicine, nursing, nutrition science, occupational therapy, ophthalmology, optometry, physician's assistant, physical therapy, psychology, rehabilitation, social work, or speech/language pathology; or approved equivalent as defined in the Department of Developmental Disabilities Rule 5123-10-04. Microsoft Office 365 including Excel, Outlook, PowerPoint, and Word required.

Microsoft Teams, SharePoint, Adobe Acrobat Pro, Early Track, and video conferencing training including GoToMeeting, Zoom, and Goggle Meet provided upon hire.

Newly hired employees must submit verification of being fully vaccinated against the COVID-19 virus.
