Job Opening

Position:            Case Manager (40 Hours per Week)

Pay Grade:         Nurse

Salary:                  Commensurate with education/experience

                                $22.00/hour - $26.00/hour

Work Hours:     Monday – Friday – 8AM – 4PM

 About the Role:

Primarily responsible for women’s cardiovascular disease risk management focusing on Southeast Ohio Counties currently served by the SE Ohio Breast & Cervical Cancer Project.  The Noble County Health Department is seeking a dedicated and compassionate Case Manager to navigate eligible women through cardiovascular risk screenings, healthy behavior support programs, and follow-up visits as needed.

Key Responsibilities:

• Establish and enroll cardiovascular health physicians as program providers
• Conduct client intake, eligibility screening, and enrollment
• Minimal travel to the target counties to recruit providers and clients
• Establish and enroll cardiovascular health physicians as program providers
• Coordinate appointments for screenings and diagnostic follow-ups
• Educate clients on cardiovascular disease and available resources
• Support clients through care navigation and referrals
• Maintain accurate and timely documentation
• Build relationships with local healthcare providers and partners

Preferred Qualifications:

• Registered Nurse (RN) license preferred
• Associate’s or Bachelor’s degree in Nursing, Social Work, Public Health, or related field
• Experience in case management, care coordination, or community health,
• Excellent communication, time management, and documentation skills
• Ability to work independently
• Proficiency in Microsoft Office and data entry systems
• Valid driver’s license and reliable transportation (occasional regional travel)
