Community Health Specialist II – Help Me Grow Program Coordinator
POSITION INTRODUCTION – GRANT FUNDED POSITION
The Ross County Help Me Grow (HMG) Home Visiting program is an evidenced-based parent support program that encourages early prenatal and well-baby care, as well as parenting education to promote the comprehensive health and development of children. The HMG Program Coordinator will utilize public health concepts and theories to ensure the promotion of healthy development of families in Ross County, Ohio who participate in the home visiting program. The position will facilitate and participate in the development, assessment, planning, budgeting, training, and evaluation efforts related to the HMG HV program. The HMG Program Coordinator will coordinate day-to-day program operations, outreach and marketing. The position will also be responsible for providing supervision for home visiting staff, monitoring program compliance per the contract with Ohio Department of Health, and ensuring model fidelity of the evidenced-based Healthy Families of America.
MINIMUM QUALIFICATIONS AND CHARACTERISTICS:
- Individual holds at least a bachelor's degree from a council for higher education accredited college or university in a field closely related to early childhood or human services, with three years of relevant experience in serving children and families; or possess a bachelor’s degree from a council for higher education accredited college or university with a minimum of three years of relevant experience working with pregnant women, infants and/or toddlers.
- Knowledge of maternal-infant health, early child development, family engagement, and concepts of child abuse and neglect
- Strong communication and public speaking skills.
- Skilled in use of computer systems, web-based software applications, and Microsoft 365.
- Organization and planning skills.
- Ability to pass a B.C.I. Background Check and a Drug Test.
- Ability to pass a background check with the Ohio central registry on child abuse and neglect.
- Individual completes a personal profile with verification on the Ohio professional registry
- Individual completes mandatory training, not limited to those required by the evidenced-based model being implemented.
LICENSURE OR CERTIFICATION REQUIREMENTS:
- Must have a valid State of Ohio driver’s license and remain insurable in accordance with the agency’s insurance policy.

General office setting in health department facilities; as well as in community sites.
The employee is exposed to, must negotiate, use, or work with or in the vicinity of chemical compounds found in an office environment (e.g. toner, correction fluid, etc.). The employee has regular contact with the public, including potentially violent or emotionally distraught individuals. Work performed in the community is sometimes subject to conditions that range from inclement weather to dangerous conditions such as snow/ice, cold, heat, noise, wetness/humidity, vibration, sudden temperature changes, and poor illumination at the job site or due to travel on the job. Travel to and from field locations may subject worker to increased risk of driving hazards. Community locations may subject worker to communicable diseases, insects and other disease vectors, toxins, hazardous materials, chemicals and animals. There will be routine exposure to blood, body fluids, and tissue.

ESSENTIAL FUNCTIONS OF THE POSITION:
- Works in collaboration with the Public Health Nursing Director to ensure effective and efficient program administration that is essential for program success.
- Facilitates and participates in development, assessment, planning, budgeting, training, and evaluation efforts related to the HMG HV program.
- Utilizes data-informed decision making in program management, oversight, and improvement planning.
- Understands and manages assigned budget and monitors program expenditures.
- Demonstrates essential knowledge and skills needed to support home visitors in their work with families, as the home visitor-supervisor relationship provides the foundation for home visiting practice.
- Uses evidence-based practices in determining needs in the field, including frequency and intensity of service delivery, caseload guidelines, and supervision needs of home visitors.
- Provides direct, ongoing, intensive, professional guidance to home visitors on a weekly basis per requirements of the HFA model.
- Implements a variety of evaluation and quality assurance mechanisms (e.g., needs assessment, qualitative and quantitative measures, record reviews, parent and staff surveys) to support program improvement.
- Accurately and clearly explains and adheres to agency, federal, state, and local laws and policies affecting documentation, confidentiality, personnel practices, and professional boundaries.
- Assures prompt and appropriate report of harm or threatened harm to the health and welfare of a child to Child Protective Services.
- Ability to identify, develop and maintain collaborative relationships with community agencies serving families, providers and other community stakeholders. Supports staff to sustain these relationships
- Promotes outreach, public awareness, and marketing to ensure families and community agencies are informed of available services.
- Actively participates in advisory group meetings and activities including program progress reporting.
- Assures quality of service, protection of integrity, and respect for the families served.
- Assists with the selection of home visiting staff and participates in orientation/training.
- Assesses and supports each home visitors ongoing needs for professional development, particularly when new knowledge and skills are required.
- Ensures that home visitors maintain a professional development plan and a record of individual professional development.
- Monitors personal needs for professional growth and seeks learning opportunities.
- Conducts records reviews, maintains data collection system, and implements a quality assurance plan.
- Provides case management oversight and monitors performance of home visiting staff.
- Prepares and submits information, reports, and invoices necessary for reimbursement for HV services.
- Manages contracts as well as the ongoing program development.
- Functions as the team leader, conducts team meetings/planning groups, provides daily support and crisis management.
- Coordinates completion of ongoing screenings and assessments described in the HMG rule.
- Coordinates information sharing with medical and service providers.
- Enters data into electronic state data base and hard copy records, spreadsheets, and databases. Completes client reporting with required timeframes.
- Develops protocols and procedures to ensure high quality service delivery; HFA accreditation status is obtained within the allotted timeframe.
- Participates in local and state meetings (e.g. regional meetings, coalition /committee meetings) as requested.
- Reports program performance to Performance Management Team and participates on quality improvement projects as assigned.
- Responds to public health emergencies as needed or determined by health commissioner or administrator (e.g. infectious disease outbreaks, mass vaccination and points of dispensing of medication). Participates in public health emergency response trainings and exercises.
- Attends and participates in agency trainings, meetings, conferences, seminar and other job-related training sessions as necessary.
- Adheres to all Ross County Health District policies, procedures and standing orders.
- Interacts effectively with other staff members.
- Seeks educational opportunities to increase skill and public health knowledge / competencies.
- Maintains required licensure and certification.
- Assures program compliance with the agency’s records retention policy.
- Meets all job safety requirements and all applicable OSHA safety standards that pertain to essential functions.
- Demonstrates regular and predictable attendance.
- Ability to work irregular hours occasionally to attend community events, meetings, or speaking engagements to meet program activities or outreach.
- Ability to work irregular hours, nights, weekends and holidays during public health and vaccine emergencies.
- Performs any and all other related duties as assigned or directed by immediate supervisor, administrator, or health commissioner.
