Background 

AOHC convened a joint committee of hospital and local health district representatives, plus HPIO, to create a toolkit for Ohio's hospitals and local health districts to assist all in meeting IRS and PHAB requirements related to CNHAs and CHAs.  This project was made possible in part through a grant from the County Health Rankings.  AOHC thanks its partners for collaborating with us to develop this resource.
