POSITION: Health Educator and Case Worker
AGENCY: Warren County Health District
DEPARTMENT: Nursing Division
LOCATION: 416 S. East Street, Lebanon, Ohio
HOURS: Full-Time (40 Hrs/Wk)
PAY RANGE: Pay Grade 8 minimum rate of 18.98 per/hr. Salary Range ($39,478-$59,612). Includes full WCHD benefits
POSTING PERIOD:   This notice is being posted for a period of time not less than seven (7) consecutive calendar days beginning 05/02/2019 and until position if filled.

MINIMUM QUALIFICATIONS: Bachelor’s degree in Social Work, Health Education, or a related field and one year relevant experience and/or training.

Knowledge, Skills and Abilities: Knowledge of psycho-social risk assessments for all clinic clients; knowledge of health education; knowledge of risk factors/areas of concern; knowledge of safety and injury prevention; and methodologies in prevention and treatment of disease; office practices and procedures; basic computer skills.

Job Responsibilities: Performs psycho-social risk assessments; provides case management; document of risk factors/areas of concern; makes necessary referrals and provides community resources. Assist clients with Medicaid enrollment, if necessary. Records accurate patient documentation into Electronic Health Records program. Acts as agency Public Information Officer. Provides health education to agency and community. May manage assistance programs, for example; Car Seat Program and Safe Sleep Program. Serves on consortiums and other community projects. Participate in Emergency Preparedness and Accreditation.
