Acknowledgment of Support: This project is being funded entirely with funds the Meigs County Health Dept. (MCHD) received from the Ohio Dept. of Health.
MCHD Mission: To preserve, promote, and protect the health and well-being of Meigs County.

Introduction: MCHD, an Equal Opportunity Provider/Employer and a political sub-division of the State of Ohio whose governing entity is the Meigs County Board of Health, seeks a contractor to conduct activities necessary to compile a complete Community Health Assessment (CHA) as per Public Health Accreditation Board (PHAB) Domain 1 standards and measures on its behalf using the Mobilizing for Action through Planning and Partnership (MAPP) model.

In addition to meeting PHAB Version 1.5 Domain 1 standards and measures, the MCHD must work with a non-profit hospital and has selected Holzer Health System with which to collaborate on the development of a regional Community Health Needs Assessment (CHNA)/CHA as required by the State of Ohio and the Affordable Care Act/IRS. The primary goal of this regional planning and strategy process is to improve the health of the populations served by all parties comprising the Regional Core Group. History: By 2020, all health departments in the state of Ohio are required by law to be accredited from the PHAB. Part of that accreditation consisted of implementing the MAPP model in our community. MCHD contracted with Ohio University's Voinovich School of Leadership and Public Affairs to assist in the initial stages and to help introduce the model to the Meigs County Health Department staff members and to conduct a CHA in 2015. Since that time,

Get Healthy Meigs! (a combined group of community stakeholders) have joined together to tackle the five priority areas that impact Meigs County most significantly. The MCHD/Get Healthy Meigs! also partnered with the University of Rio Grande to produce the 2017-2022 Meigs County Community Health Improvement Plan (CHIP).

Scope of Contracted Work Sought:

  1. Facilitate four MAPP assessments as explained in the framework for Meigs County via Get Healthy Meigs!. http://archived.naccho.org/topics/infrastructure/mapp/framework/index.cfm
  2. Participate in MAPP assessments conducted by the Regional Core Group; share information and attend meetings held by the Regional Core Group as agreed upon and as needed to Identify and select at least two priority topic areas from the Ohio State Health Improvement Plan (https://www.odh.ohio.gov/-/media/ODH/ASSETS/Files/chss/ship/SHIP_02072017.pdf?la=en) to address regionally; select priority outcome indicators to track; select cross-cutting strategies and indicators to measure impact in each of the following factors: Social Determinants, Public Health and Prevention, Healthcare Access; review periodically progress in each initiative; reassess and realign as necessary.
  3. Become familiar with and adhere to PHAB Version 1.5 Domain 1 standards and measures (http://www.phaboard.org/).
  4. Compile data from available data sources (primary and secondary) on demographics, health disparities and County Health Rankings; aggregate and compile a report.
  5. Conduct an environmental scan; compile a report.
  6. Provide convenience survey tools; assess results and compile a report.
  7. Provide technical assistance to gather resident data; create a script, facilitate one focus group and report on main themes.
  8. Attend and participate in Get Healthy Meigs! Meetings for the duration of the project.
  9. Provide Implement a CHA as informed by the MAPP assessments; PHAB Version 1.5 Domain 1 standards and measures; data collection; main themes identified by focus group; environmental scan; convenience survey. Analyze data, trends and emerging issues and compile a report; provide recommendations as appropriate based on findings.
  10. Ensure preliminary findings of the assessment were distributed to the community at large and seek the community’s input.
  11. Inform partners, stakeholders, other agencies, associations, and organizations of the availability of the community health assessment; provide an overview/summary report of the overall CHA to be shared with the public.

MCHD Responsibilities:

  1. Share information and participate in conference calls/meetings as needed.
  2. Compile and share internal documents and agency data as requested by the contractor.
  3. Review draft materials produced by the contractor.
  4. Coordinate Get Healthy Meigs! Meetings; communicate with and recruit stakeholders and provide/arrange for meeting space for stakeholders as necessary.
  5. Conduct convenience surveys via contractor-provided tool.
  6. Provide clerical support for and during Get Healthy Meigs! Meetings.

A decision will be made by or before 10/15/18. All those who apply will be notified about the decision not just the successful applicant. Decisions of the MCHD/Board of Health are final.

The Project Start Date will be proposed by the applicant and mutually agreed upon with the MCHD. The Project Completion Date will be 6/30/19 (including submission of all CHA reports/materials to the MCHD).
