ACCREDITATION COORDINATOR

ASHLAND COUNTY GENERAL HEALTH DISTRICT

POSITION DESCRIPTION

The Accreditation Coordinator (AC) will oversee the Ashland County General Health District’s process for developing and submitting documents as part of the application process for public health accreditation through the Public Health Accreditation Board (PHAB). The Accreditation Coordinator will provide guidance to staff on how to interpret PHAB Standards and Measures and will facilitate regularly scheduled meetings of staff teams to collect and/or develop the documents. This will include development of agency plans to improve health within the community and improve quality in service delivery. This qualified individual will maintain a database of documents that demonstrate conformity to accreditation standards and measures and serve as a liaison between the Ashland County General Health District and any other partnering agency that is contributing to the accreditation process. The AC will be the primary point of contact during the entire application, documentation collection and review, site visit, and reaccreditation efforts through PHAB.
